The scope of practice privileges for a pharmacist in both outpatient and inpatient settings includes evaluation and interpretation of physician's orders and prescriptions (written, electronic, and/or verbal) for appropriateness of drug therapy (indication, dosing, dosage formulation, contraindications, patient’s age and co-morbid conditions, etc.); monitor patient’s drug therapy for efficacy, drug interactions, and adverse reactions; and make proper recommendations for drug-therapy adjustments/modification. Pharmacist will review of proper drug selection, subtherapeutic dosages, overdosages, adverse drug reactions, and drug interactions in the inpatient and outpatient settings. Pharmacist will document pharmaceutical care activities in the medical record as appropriate. Pharmacists prepare, compound, and dispense pharmaceuticals for patients of all ages in the inpatient and outpatient setting, conduct education for patients and healthcare professionals, and ensure proper handling and dispensing of investigational drugs. Works under general supervision of the Chief, Pharmacy Director who discusses policies and objectives. There is no direct supervision, and the individual exercises independent judgment in selection and performance of the full range of diagnostic evaluations of patients.

  • Assess patient’s response to drug therapy and plan drug therapy based on physician-established diagnoses.
  • Order and provide interpretation of laboratory tests necessary to evaluate drug therapy effects and outcomes.
  • Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff.
  • Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotics).
  • Initiate or modify drug therapy for minor acute conditions such as colds, rashes, and allergies.
  • Administer prescription or non-prescription drugs according to established agreements or protocols.
  • Assess metabolic needs and ordering therapeutic enteral or parenteral nutrition products in the inpatient setting.
  • Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly. Consulting with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.
  • Conduct and coordinate clinical investigations and research (consistent with other healthcare professionals) approved by a local or regional Investigational Review Board (IRB) and participate in outcome studies generated by the department of pharmacy and approved by the P&T Committee.
  • Perform a full range of pharmacist tasks/procedures.
  • Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.
  • Counsel patients regarding appropriate use of medications.
  • Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
  • Ensure prescription directions are clear and accurate in regards to use, dose and frequency. Also check for medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data that is appropriate for individual patient safety. Intervention with providers shall be made for noted discrepancies of above listed items.
  • Compound medications as required.
  • Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Work closely with clinical staff in developing patient wellness programs such as smoking cessation programs and asthma classes.
  • Provide medication information and therapy guidelines to the clinical staff as required.
  • Provide medication and healthcare related in-service training to clinical staff on new medications on the market covering topics such as: appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
  • Provide clinical direction to pharmacy personnel.
  • Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations.
  • Supervise drug/stock integrity and inventory i.e. drug recalls, drug inventory storage within the pharmacy and clinics governed by the military treatment facility (monthly clinic inspections).
  • Consult with other specialty practitioners to provide optimal patient pharmaceutical care.
  • Ensure that work areas are clean and safe according to applicable regulations.
  • Document significant medication interactions, and pharmacy interactions with prescribers. Conduct Adverse Drug Reaction (ADR) reporting in accordance with TJC or AAAHC standards as appropriate.
  • Facilitate medical staff drug utilization reviews (DURs) as deemed necessary.
  • Participate in established peer (other medical specialist) review process monthly. Provide chart review for quality assurance purposes in the area of clinical pharmacy if requested on an occasional basis.
  • Attend and participate in patient care reports, patient care conferences, team conferences, professional staff conferences and other appropriate professional activities only to the extent that such attendance and participation is relative to his/her assigned cases and/or performance of services as determined by the supervisor.
  • Provide instruction for PharmD students in the form of clinical rotations as requested.
  • Closely monitor medication therapy by reviewing patient profiles at the time of dispensing.
  • Maintain regular and punctual attendance.
  • Other duties as assigned.

Physical Demands:

Position consists of sitting for long periods of time, using hands/fingers, handling, reaching with hands and arms, speaking or translating. Periods of walking, standing, bending, kneeling, stooping, crouching, and lifting boxes weighing up to 30 pounds. Moderate to extended periods of office work with heavy periods of computer usage. May be assigned occasional to moderate periods of analytical tasks requiring extensive reasoning abilities. Work Environment:

Work is performed primarily indoors at a well-lit military treatment facility such as a clinic, outpatient facility or hospital. The environment is normally air conditioned, but conditions may change dependent upon circumstances. Work may need to be performed in a fast-paced environment requiring quick thinking and rapid judgements. Employee will be exposed to a wide variety of clients in differing functions, personalities and abilities. Exposure to bloodborne pathogens and bodily fluids, latex, radiation or other contaminants and safety issues. May be exposed to infection or diseases from contact with patients. Work is highly social with patients and can be stressful with a high emotional element. Decisions made may substantially impact patients and their families.

Work Schedule:

Full-time. May be required to work daytime, evening, weekend shifts and/or extended work hours to successfully complete project or assignment.

Job Requirements

Minimum Qualifications:

  • Education: A Bachelors, Masters, or Doctoral degree in Pharmacy from a College of Pharmacy accredited by the Accreditation Council of Pharmacy Education (ACPE).
  • Post-Graduate Training: Not required but desired.
  • Specialty Board Certification: Not required but desired.
  • Licensure: Must have current, valid, unrestricted license to practice pharmacy in any one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
  • Experience/Clinical Competency: A minimum of one year professional pharmacy experience as a licensed pharmacist and has the ability to execute assigned tasks and practice pharmacy independently and perform sound clinical judgment. Must not have narcotic registration suspended or revoked.
  • Health Status: No medical or mental health problems which could affect work performance and patient care.
  • Must have and maintain current certification in basic life support (BLS) from the American Red Cross. This certificate must be current at all times.
  • Professional Behavior/Conduct: Must not be a defendant or the subject of an active malpractice action or felony case. Must not have reported allegations of misconduct to external regulatory agencies where there is reasonable cause (such as an arrest) that could have a serious impact on patient/staff welfare or safety.
  • Must obtain and maintain the site's required health screening and immunization requirements.
  • Must obtain and maintain any necessary security access and/or background checks.
  • Must obtain and maintain all required documents, certifications, licenses, etc. listed on the credentialling checklist, sent by Katmai’s Credentialing Specialist, for this position for initial hire.

Special Skills:

  • Fundamental knowledge of state and federal laws, regulations, standards, and requirements governing drug management and dispensing.
  • Fundamental clinical/drug therapy knowledge to assess patients for over-the-counter medications, emergency and bridge prescription needs.
  • A working knowledge of pharmacy computer systems and automations such as electronic health record i.e. CHCS, AHLTA, Essentris (or equivalent), Innovations PharmAssist, GSL, etc.
  • A working knowledge of DoD/DHA prescription benefits program/process.
  • Ability to work as a member of a team when performing procedures that require making rapid and accurate observations or adjustments to established practices.
  • Must be able to read, understand, speak, write English fluently.
  • Excellent written communication skills to perform accurate documentation.
  • Ability to effectively communicate and collaborate with a diverse group of people.

CSS Compensation and Benefits package:

  • Pay Range: $54.00-$57.00 per hour based on experience
  • SCA Health and Welfare: $4.22/hr paid toward employee only medical insurance. Any remainder paid in cash on bi-weekly paycheck.
