We are searching for an Outpatient Pharmacist with inpatient experience to support Langley AFB located near Hampton, VA. The outpatient technician to pharmacist ratio is 1 pharmacist to 3 technicians. The outpatient hospital hours are currently 7:30AM to 5PM daily, the Satellite pharmacy is open from 9AM to 1PM every Saturday. Inpatient coverage may be needed on occasion. The hospital has 35 beds and will be increasing to 80 beds when the new hospital opens at the end of the Summer. Patients consist of mostly Labor/Delivery, nursery beds, post surgery patients, and ER admitted patients to the ward or ICW. The hospital pharmacy prepares 700-800 prescriptions per day provider order entry. Inpatient Pharmacist ratio is 1 pharmacist to 1-2 technicians. Inpatient hours are 6AM-8PM. The pharmacist will be scheduled 9 hours per day with 1 hour uncompensated meal break.
SPECIFIC TASKS
Pharmacist will have a working knowledge of pharmacy computer systems such as CHCS computer system.
Perform a full range of pharmacist procedures.
Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.
Counsel patients regarding appropriate use of medications.
Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancy of above listed items.
Compound medications are required.
Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
Work closely with clinical staff in developing patient wellness programs such as smoking cessation programs and asthma classes.
Provide medication information and medication therapy guidelines to the clinical staff as required.
Provide medication and healthcare related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
Provide clinical direction of corpsmen and pharmacy technicians and provide input into job performance evaluations.
Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulation.
Consult with other specialty practitioners to provide optimal patient pharmaceutical care.
Ensure that work ares are clean and safe according to applicable regulations.
Document significant medication interactions and pharmacy interactions with prescribers. Conduct Adverse Drug Reaction (ADR) reporting in accordance with JCAHO standards.
Facilitate medical staff drug utilization reviews (DURs) as deemed necessary.
REQUIREMENTS
A Bachelors, Masters, or Doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
A current, valid, unrestricted license to practice Pharmacy in any of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
Experienced as a registered pharmacist for at least 24 consecutive months within the last 36 months.
