Maxim Government Services is looking for qualified Inpatient Pharmacists for our client, the West Haven, CT VA Medical Center for full time overnight, weekend, and holiday coverage. 10 and/or 8 hour shifts available, 37-40 hours/week.
1. Provide on-site verification and processing of inpatient medication orders and discharge prescriptions for VA Connecticut.
2. Assure that accuracy of verification of all medication orders (inpatient, discharge) shall be 100%.
3. Verify medication orders within 30 minutes or less of electronic receipt of order.
4. Assure that accuracy of all tasks performed by the pharmacy technician working with the pharmacist during these hours shall be 100%.
5. Be required to correct all orders (dosing, appropriateness, duration and instruction, compliance with the local, VISN and national formulary guidelines and restrictions) and document all interventions.
6. Conduct accurate medication reconciliation, particularly upon discharge of the patient from the hospital.
7. Be required to provide accurate and timely drug information and calculate doses based on pharmacokinetics of specific drugs. Must be able to calculate doses based on renal function, fluid restriction and other pharmacodynamic states of patients.
8. Be knowledgeable of and proficient in the preparation of sterile IV products (sterile technique) and requirements of Chapter 797.
9. Be able to provide accurate information regarding formulary and non-formulary agents to providers. Must be capable of enforcing formulary management initiatives and must be proficient in recommending use of formulary agents in place of non-formulary agents.
10. Be able to fill prescriptions accurately and timely.
11. Be able to come in during the day hours on occasion to attend training sessions or important staff meetings.
Requirements:
(1) Citizenship. Citizen of the United States.
(2) Education. Graduate of a degree program in pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP).
(3) Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
Creditable Experience:
(a) Knowledge of Current Professional Pharmacy Practices. To be creditable, the experience must have required the use of knowledge, skills, abilities and other characteristics associated with current professional hospital pharmacy practice. The hospital experience or pharmacy education must have been gained within 3 years prior to the date of appointment. This may be evidenced by one or more of the following during the 3-year period:
1. The equivalent of 1 year of active professional practice in a hospital pharmacy practice setting. Active professional practice means paid/non-paid employment as a professional pharmacist as defined by the appropriate licensing board; or
2. In addition to a full, current and unrestricted license, annual completion of a minimum of 15 continuing education units (CEUs) recognized by ACPE; or
3. Academic course work leading to an advanced degree in pharmacy or a related health care field.
(b) Crediting Residency and Fellowship Training as Experience
1. Pharmacy degree programs have changed over time from the traditional bachelor’s four-year degree to the 6-year Pharm.D. or formal post-baccalaureate (Pharm.D.) hospital oriented degree program as recognized by the American Council on Pharmaceutical Education (ACPE).
2. After completing the degree requirements, many pharmacists seeking hospital positions complete a one-year PGY-1 (and occasionally a one-year PGY-2) clinical pharmacy residency program or a two –year pharmacy fellowship program (particularly those pharmacists interested in research positions). This post-degree training is creditable as experience on a year-for-year basis
3. A hospital residency recognized by the American Society of Health-System Pharmacists (ASHP) does not include internships which are shorter training programs designated to meet the requirements of boards of pharmacy for licensure. Hospital residencies may be substituted for experience on a year-for-year basis. There are two levels of residency. The Post Graduate Year 1 (PGY1) is a general practice residency. The Post Graduate Year 2 (PGY2) residencies are in specialized areas of pharmacy practice.
4. Fellowship programs are typically 2-year training programs in a specialized area of clinical practice. Applicants for these programs usually possess a Pharm.D. or an equivalent degree and have completed an accredited clinical or specialized residency (or both). Training as a fellow may substitute for creditable experience on a year-for-year basis.
(c) Quality of Experience. Experience is only creditable if it is post-licensure experience as a professional pharmacist directly related to the position to be filled. In this case, the experience must be hospital pharmacy experience.
Orientation and Training:
The majority of the work hours during this period of orientation and training will be eight (8) hour shifts between the hours of 7AM and 6PM, Monday through Friday.
As the training progresses, some of the work days will shift to Saturday’s and Sunday’s to familiarize the pharmacist with the work patterns of the weekend.
The final stage of orientation and training will include assignment to the evening shift with the work hours of 3:30PM – 12 AM to familiarize the pharmacist with the work patterns of the evenings.
