Job Description

POSITION SUMMARY
The Clinical Pharmacist performs fundamental clinical pharmacist duties that enable the delivery of optimal pharmacotherapy to patients and facilitates safe, efficacious, and pharmacoeconomically rational use of medications in the population(s) served. This involves daily reactive and targeted proactive medication regimen reviews of targeted medication therapies and patients within one’s designated area(s) of responsibility, supporting the delivery of pharmacy managed protocols, formulary processes and medication reconciliation activities while providing education to fellow pharmacists, nursing and medical staffs. The Clinical Pharmacist is active with precepting doctor of pharmacy students on advance pharmacy practice experiences.

EDUCATION REQUIREMENTS
Must have a Doctor of Pharmacy degree or equivalent and have successfully completed an ASHP-accredited post-graduate year 1 (PGY1) residency training program or have demonstrated equivalent professional experience.


EXPERIENCE REQUIREMENTS
In addition to demonstrated excellence in operational pharmacy practice successful candidates will have a demonstrated record of experience providing fundamental clinical pharmacy services and demonstrate experience with direct patient care activities and interactions with nursing and medical staff members. Candidates having not completed a PGY1 residency program will be required to demonstrate experience commiserate to that achieved within an ASHP-accredited PGY1 residency training program (i.e. completion of HMH’s departmental clinical staff development program and minimum of 3 years of HMH practice experience).

CERTIFICATES, LICENSES AND REGISTRATIONS REQUIRED
•Must be licensed or eligible for licensure to practice pharmacy in the State of Texas.
•Must maintain continuing education credits to maintain professional licensure.
•Must maintain a current Texas pharmacist preceptor license (or must obtain within 12 months of start of employment)
•BLS & ACLS (*PALS Certifications required for pharmacists regularly assigned to respond to code blue events. Refer to department manager)

SPECIAL KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
•Must operate at a proficient level with regards to one’s application of the principles of rational drug therapy including but not limited to: application of current evidence-based guidelines, novel and innovative clinical practices).
•Must possess excellent skills in supervising and training peers, students, and supportive personnel.
•Must possess excellent verbal and written communication skills in English.
•Must be proficient in the use of personal computers and related software.

WORKING ENVIRONMENT
•Must be familiar with Body Substance Isolation and Blood Borne Pathogen guidelines.
•Must be familiar with the OSHA guidelines for the safe handling of cytotoxic and hazardous substances.
•Must satisfy the Hazard Communications Act requirements.
